The Referral Form will be submitted as an attachment in LoanWell for each business referred for the EFOD Collaborative Lending Program. It will be a checklist to ensure that all the criteria (as set by EFOD Collaborative) were met for this program along with any other relevant information that is needed by Community Credit Lab.
